Types of Licenses

Charitable Organization
nonprofit board, association, corporation, or other organization domiciled in this state and qualified with the United States Internal Revenue Service for an exemption from federal income tax under Section 501 (c), (3),(4),(5),(6),(7),(8),(10) or (19) of the Internal Revenue Code.

Pull-Tabs
Single or banded tickets or cards each with its face covered to conceal one or more numbers or symbols, where one or more card or ticket in each set has been designated in advance as a winner.
Fee included in organizational license fee.
Raffle
A type of lottery in which several persons pay, in shares, the value of something put up as a stake, and then determine by chance which one of them shall become the sole possessor of it and any portion or share is retained by the person(s) conducting the raffle.

Special Session
A license to conduct one bingo session where the total prize amount shall not exceed $25,000 in cash or things of equal value. A Special Session is limited to six consecutive hours. No organization shall be issued more than two special licenses per year. A current officer approved by the Office must complete and sign the Special (Super) Bingo License form. This form must be submitted to the Office for approval, along with the correct fee paid by check drawn from the Charitable Gaming Account, no later than 72 hours prior to the session.
Commercial Lessor
Any person or other entity other than bona fide non-profit organizations licensed under this Chapter who leases any building, structure, or premises to organizations.
Non-Commercial Lessor
A bona fide non-profit organization licensed by the Office to conduct games of chance which leases any building or structure used for charitable gaming to other organizations licensed by the Office.
Distributor
Any person or other entity who sells, offers for sale, or otherwise furnishes to any person supplies or equipment for use in the conduct of any game of chance authorized under this chapter and any private contractor qualified to conduct games of chance as authorized.
Manufacturer
Any person or other entity who manufactures for sale, offers for sale, or otherwise furnishes any gaming supplies or equipment for use in the conduct of any game of chance.
Electronic Video Bingo
A machine designed for the specific purpose of playing the game of bingo that has an electronic random-number generator to select numbers in lieu of the drawing of numbers from a receptacle and that one or more video images containing numbers or other designations five or more in one line may be utilized instead of a card.
Electronic Video Bingo Manufacturer
Any person or business entity who manufacturers for sale electronic video bingo machines in this state.
Electronic Video Bingo Distributor
A person or business entity who owns and leases electronic video bingo machines to a charitable organization in this state.

Changes to License Application

Modification

Any addition, deletion, or other change to an organization's charitable gaming license.

The Office will issue a new license after each change and the new license must be displayed in the hall where games are played. If you do not receive the new license prior to the session, please contact the Office.

Session Change

A current officer, approved by the Office, must complete and sign the location schedule change form. This form must be submitted to the Office no later than 72 hours prior to the change. If the organization is taking a session from another organization, the session requested will need to be deleted from the other organization's license and added to the organization requesting approval for the added session. Both organizations will need to submit a location schedule change form in order for the Office to approve the session change.

The Office will issue a new license after each change and the new license must be displayed in the hall where games are played. If you do not receive the new license prior to the session, please contact the Office.

Adding Officer(s)

Organization Official Information Sheet must be completed and submitted within ten days of the change in officer(s). A current officer, approved by the Office, must sign the form authorizing the change. List at least one official as Member-in-Charge (MIC) and as many alternate Members-in-Charge as necessary to assure that at least one MIC is present at all games as provided by LA.R.S. 4:714 (D).

The Officer(s) must complete the section titled explanation attached. Officer(s) must complete the yes or no box. Yes, if he has been arrested. No, if he has never been arrested. If yes is marked, attach an explanation.

Also, if an officer is no longer with the organization, a current officer/MIC must inactivate the officer within ten days of the change by submitting a signed letter to the Office.

Deleting Officer(s)

Organization Official Information Sheet must be completed and submitted within ten days of the change in officer(s). A current officer, approved by the Office, must sign the form authorizing the change. Change in Member(s)

To add or inactivate a member, an official can submit the request in writing by mail, fax or on a diskette. The request must contain the following information: the organization's license number, the complete home address of the member including zip code, the member's social security number and full date of birth (month, date and year). There is no charge for changes in members.

Email is not a secure environment, so never send confidential information (i.e. Social Security numbers, tax account numbers) through Email. To protect your privacy, we will not send sensitive or confidential information to anyone by means of Email.

Withdrawal
An organization is ceasing playing Bingo and wishes to cancel its license.
If an organization is withdrawing from gaming, the request must be submitted to the Office in writing and must be signed by a current officer. The withdrawal request must specify the last date a gaming session was held, along with an inventory of supplies on hand. The organization must surrender the original license, make sure all fees are current, and file a final quarterly report with the Office.
