Electronic Video Game Devices

To obtain approval for Electronic Video Gaming (EVG) devices in Louisiana, follow these steps:

1. The manufacturer must complete the Video Gaming Device Shipment Notification (Form OCG223).

   • The application must be signed by an authorized manufacturer representative.
   • A bill of sale must be included with the application.

2. The Office of Charitable Gaming will review the shipment notification.

   • If the distributor and shipping location are properly licensed, the Office will approve the shipment.
   • An approval notice will be sent to the manufacturer and must accompany the devices during shipment.

3. The distributor must complete a Video Gaming Device Permit Application (Form OCG221) for each device.

   • A non-refundable permit fee must be submitted with each application.
   • The fee may be paid as:
     • $600 per device, or
     • $50 per month for the applicable months prior to the application.

4. Before any device may be placed into operation, the Office of Charitable Gaming must inspect, test, and permit each device.
