Electronic Video Game Devices
To obtain approval for Electronic Video Gaming (EVG) devices in Louisiana, follow these steps:

The manufacturer must complete the Video Gaming Device Shipment Notification (Form OCG223).
- The application must be signed by an authorized manufacturer representative.
- A bill of sale must be included with the application.

The Office of Charitable Gaming will review the shipment notification.
- If the distributor and shipping location are properly licensed, the Office will approve the shipment.
- An approval notice will be sent to the manufacturer and must accompany the devices during shipment.

The distributor must complete a Video Gaming Device Permit Application (Form OCG221) for each device.
- A non-refundable permit fee must be submitted with each application.
- The fee may be paid as:
- $600 per device, or
- $50 per month for the applicable months prior to the application.

Before any device may be placed into operation, the Office of Charitable Gaming must inspect, test, and permit each device.
