Private Casino Contractor

To become a licensed Private Casino Contractor in Louisiana, follow these steps:

1. Complete the Private Casino Contractor License Application (Form OCG2107).

   • The signature date and notary date must match.

2. Complete Page 2 of the application for each:

   • Officer
   • Board Member
   • Owner with more than 5% ownership

   Each individual must also submit:

   • Personal History (Form OCG212)
   • Personal Financial History (Form OCG213)

   Note: Social Security numbers are required for all applicable individuals.

3. Submit the completed application and all required documentation to the Office of Charitable Gaming.

4. The Office will review your application. An investigator will contact you to:

   • Schedule a site inspection
   • Answer any questions regarding your application.

5. After the inspection, your license will be issued unless additional information is required. If additional documentation is requested, the license will be issued once all requirements have been satisfactorily met.
